Role Overview
As a Physical Therapist, you will be responsible for evaluating patients, developing and implementing individualized care plans, and helping patients regain mobility, strength, and independence in the home setting.

Primary patient populations may include:

• Post-surgical and orthopedic recovery
• Neurological conditions
• Chronic conditions requiring mobility and strength support

Key Responsibilities

• Perform in-home physical therapy evaluations and re-evaluations
• Develop and execute individualized treatment plans
• Document patient progress and outcomes accurately and timely
• Educate patients and caregivers on exercises, safety, and mobility
• Collaborate with nurses, physicians, and the broader care team
• Ensure care aligns with physician orders and regulatory standards

Qualifications

• Active Physical Therapist license in the state of Illinois
• Graduate of an accredited Physical Therapy program
• Home health experience preferred, but not required
• Strong clinical judgment and patient-centered mindset
• Reliable transportation and ability to travel within McHenry and Kane County
